Emergency Notice
A-LIST App is not an emergency service. If you are experiencing a medical emergency, a life-threatening condition, severe symptoms, or thoughts of harming yourself or others, call 911 immediately or go to the nearest emergency room. If you are in emotional distress or suicidal crisis, call or text the 988 Suicide & Crisis Lifeline at 988.
Do not use A-LIST App for urgent or emergency medical needs. Messages, forms, chats, emails, calls, or other communications through A-LIST App may not be reviewed immediately.
Consent to Telehealth
By clicking "I Agree," checking an acceptance box, creating an account, using A-LIST App, submitting information through A-LIST App, or otherwise indicating acceptance, you confirm that you have read, understood, and agreed to this Telehealth Consent.
If you do not agree to this Telehealth Consent, do not use A-LIST App for healthcare services.
You also represent that you are accepting this Telehealth Consent for yourself or that you have legal authority to accept it on behalf of the person receiving services.
Purpose of This Consent
This Telehealth Consent explains how healthcare services may be provided through A-LIST App using telehealth technologies. It describes potential benefits, risks, limitations, privacy considerations, and your rights when receiving care remotely.
In this document:
- "A-LIST App," "we," "us," or "our" means Immortal Company Inc. and the A-LIST App platform.
- "Provider" means a licensed physician, nurse practitioner, physician assistant, licensed counselor, or other licensed healthcare professional, as applicable, who provides or supports healthcare services through A-LIST App.
- "You" or "your" means the person using A-LIST App or receiving healthcare services through A-LIST App.
What Telehealth Means
Telehealth is the delivery of healthcare services when you and your Provider are not in the same physical location. Telehealth may include secure questionnaires, intake forms, messaging, email, phone calls, video visits, photo uploads, medical-record review, lab review, prescription management, remote monitoring, patient education, follow-up care, and other electronic communications.
Telehealth may be used for evaluation, diagnosis, treatment, medication management, follow-up, care coordination, and education, depending on your condition, your location, applicable law, and your Provider's clinical judgment.
Provider Credentials
Healthcare services made available through A-LIST App are provided by licensed healthcare professionals acting within the scope of their licenses, certifications, training, and professional judgment. Provider credentials may be made available to you before or during care, where required by law or operationally available. If you have questions about a Provider's credentials, you may contact A-LIST App support or ask the Provider directly.
Service Availability
Telehealth services through A-LIST App are available only in states and jurisdictions where the applicable Provider is licensed, certified, registered, or otherwise authorized to provide care, where the requested service is permitted by law, and where A-LIST App makes the requested service available. You may be asked to confirm your current physical location before receiving care. A Provider may decline or stop a telehealth encounter if your location, medical condition, applicable law, or another factor prevents the Provider from safely or legally providing care through A-LIST App.
Alternative Care Options
You may choose to seek in-person care instead of telehealth at any time. Telehealth may not be appropriate for every condition, symptom, diagnosis, treatment, or medication request. Your Provider may determine that you need in-person care, urgent care, emergency care, specialty care, laboratory testing, imaging, or another service outside A-LIST App.
Potential Benefits of Telehealth
Telehealth may offer benefits such as:
- Easier access to licensed Providers.
- More convenient scheduling and follow-up.
- Reduced need for travel or in-office appointments.
- Faster exchange of relevant health information.
- Ability to communicate with Providers through secure digital tools.
- Support for ongoing care, medication management, and patient education where clinically appropriate.
No specific benefit, outcome, diagnosis, prescription, treatment, or result is guaranteed.
Potential Risks and Limitations
Telehealth has risks and limitations. By using A-LIST App, you understand and accept that:
- A Provider may not be able to perform a physical examination or collect vital signs in the same way as an in-person visit.
- The information available to a Provider may be incomplete, inaccurate, delayed, or affected by what you choose to provide.
- A Provider may determine that telehealth is not appropriate for your needs.
- Technology may fail, malfunction, be unavailable, or produce incomplete, inaccurate, corrupted, delayed, or lost information.
- Internet connections, devices, software, cameras, microphones, notifications, or messaging systems may not work as intended.
- Delays in evaluation or treatment may occur if communications are not received or reviewed promptly.
- Electronic communications may carry privacy or security risks, even when reasonable safeguards are used.
- A Provider may have limited ability to diagnose, treat, prescribe, or manage certain conditions remotely.
- Prescribing decisions may be limited by federal law, state law, pharmacy rules, clinical guidelines, medication safety considerations, or the Provider's professional judgment.
- You may experience side effects, adverse reactions, allergic reactions, medication interactions, lack of improvement, worsening symptoms, or other unexpected outcomes.
- If you withhold or provide incomplete or inaccurate information, your care may be affected.
Your Responsibilities
You agree to:
- Provide truthful, accurate, complete, and current information about your identity, location, health history, symptoms, medications, allergies, medical conditions, prior care, and other relevant information.
- Update A-LIST App and your Provider if your information changes.
- Follow your Provider's instructions and ask questions if you do not understand them.
- Seek emergency or in-person care when directed or when your symptoms require it.
- Use A-LIST App only for yourself unless you are legally authorized to act for another person.
- Keep your account credentials secure and notify us if you believe your account has been accessed without authorization.
- Use a private location and secure device whenever possible when communicating about your health.
Provider Discretion
Your Provider is responsible for exercising independent clinical judgment. A Provider may decline to diagnose, treat, prescribe, renew a prescription, or continue care through A-LIST App if the Provider determines that telehealth is not clinically appropriate, legally permitted, or medically safe.
Using A-LIST App does not guarantee that you will receive a prescription, a particular medication, a particular dose, a laboratory order, a diagnosis, or a specific treatment plan.
Prescriptions, Pharmacies, and Labs
If a Provider determines that a prescription, pharmacy service, laboratory test, or other third-party service is appropriate, A-LIST App may help coordinate that service where available and permitted by law.
You understand that:
- Prescriptions are issued only when clinically appropriate and legally permitted.
- Controlled substances, narcotics, or other restricted medications will not be prescribed through A-LIST App unless expressly permitted by applicable law, the Provider's clinical judgment, and A-LIST App's then-current prescribing policies.
- A-LIST App may use pharmacy partners to fulfill prescriptions and ship medications directly to you when available and authorized.
- Where required by law or supported by A-LIST App's then-current operations, you may request that an eligible prescription be sent or transferred to another pharmacy of your choice.
- Pharmacy, laboratory, shipping, payment, insurance, and fulfillment services may be provided by third parties.
- Pharmacy availability, transferability, medication availability, pricing, shipping times, refill eligibility, and fulfillment are not guaranteed.
- You are responsible for reviewing medication instructions, warnings, contraindications, and questions with your Provider or pharmacist.
Privacy and Security
A-LIST App uses administrative, technical, and physical safeguards designed to protect personal information and health information. However, no electronic system, device, communication method, or storage method can be guaranteed to be completely secure.
By using A-LIST App, you understand that your information may be collected, used, stored, transmitted, and disclosed as described in applicable privacy notices, the A-LIST App Privacy Policy, and as permitted or required by law. This may include use and disclosure for treatment, payment, healthcare operations, customer support, legal compliance, safety, and other purposes described in applicable policies.
You understand that email, SMS/text messages, push notifications, phone calls, and other communication tools may involve privacy risks. Standard message and data rates may apply. You should avoid sharing sensitive information through insecure channels unless you accept those risks.
Communications and Response Times
A-LIST App may allow communications through secure messaging, forms, email, phone, video, or other tools. These communications are not intended for emergencies or urgent needs.
A Provider or support team member may not review or respond to your communication immediately. If your condition changes, worsens, or becomes urgent, seek in-person medical care, urgent care, emergency care, or call 911.
Records and Documentation
Your telehealth interactions may become part of your medical record or service record, as applicable. You understand that information submitted through A-LIST App may be used by Providers and authorized personnel to evaluate, document, coordinate, and support your care.
Unless expressly authorized by all parties and permitted by law, you may not record telehealth visits, calls, or communications.
Payment and Insurance
You are responsible for fees, charges, copayments, prescriptions, labs, subscriptions, shipping, or other costs associated with your use of A-LIST App, unless otherwise stated in writing.
A-LIST App is a cash-pay and self-pay service. A-LIST App does not bill Medicare, Medicaid, TRICARE, any federal healthcare program, any state healthcare program, private insurance, employer health plans, or other third-party payers for telehealth services. You are responsible for all charges associated with your use of A-LIST App.
By using A-LIST App, you understand and agree that services, medications, labs, subscriptions, shipping, or other related costs may not be reimbursable by insurance or other benefit programs. You should not submit A-LIST App charges for reimbursement unless A-LIST App expressly provides written documentation stating that a charge is eligible for reimbursement.
Withdrawal of Consent
You may withdraw your consent to telehealth at any time by providing written notice to A-LIST App or your Provider at support@a-list.com.
Withdrawal of consent will not affect actions already taken in reliance on your prior consent. If you withdraw consent, you may no longer be able to receive healthcare services through A-LIST App because the platform may not offer in-person care.
State Licensing and Complaints
Providers are licensed by the applicable professional licensing boards. If you have a concern about a Provider, you may contact the medical board, nursing board, pharmacy board, behavioral health board, or other applicable licensing authority in your state.
Acknowledgment and Agreement
By accepting this Telehealth Consent, you acknowledge and agree that:
- You have read and understand this Telehealth Consent.
- You have had the opportunity to ask questions about telehealth.
- You consent to receive healthcare services through telehealth using A-LIST App.
- You understand the potential benefits, risks, and limitations of telehealth.
- You understand that telehealth is not appropriate for emergencies.
- You understand that no diagnosis, treatment, prescription, medication, outcome, or result is guaranteed.
- You agree to provide accurate and complete information.
- You agree to seek emergency, urgent, in-person, or specialty care when needed or when directed by a Provider.
- You understand that Providers may decide telehealth is not appropriate for your condition.
- You understand that privacy and security risks may exist when using electronic communications.
- You understand that you may withdraw this consent as described above.
